This service does not include any additional texts, emails, visits or concierge services. This would include second-opinion services and diagnostic consultations. It includes referrals for laboratory, radiologic and special diagnostic studies. It includes consultative discussion regarding specialty referral. This does not include follow-up medical management of any conditions discussed or diagnosed in the course of evaluation. (Patient would need to elect concierge service if ongoing medical management were desired.)
