Receive ongoing, personalized care tailored to your schedule. Enjoy consistent access to your provider, all through a simple monthly subscription.

Initiation fee: $2695, which includes the first month’s subscription.

Comprehensive Initial Consultation:

Full medical history review and physical examination
Baseline lab work, including a complete blood panel and urinalysis
Electrocardiogram (EKG) to assess heart health

Ongoing Preventive & Routine Care:

Up to 12 office visits per year for check-ups, follow-ups, and general medical concerns
Management of common and chronic conditions such as high blood pressure, diabetes, asthma, obesity, autoimmune disorders, and chronic lung disease
Medication management and lifestyle recommendations to support long-term health

Advanced Diagnostic & Preventive Screenings:

Referrals for specialized tests, including:
- Cardiac health – nuclear stress testing, pulmonary function testing
- Cancer screening – mammography, colonoscopy, PSA testing, Cologuard
- Full-body scans for early detection of potential health issues

Coordinated Specialist Care:

Referral access to top medical specialists as needed for further evaluation and treatment

This service includes initial diagnostic consultation and follow-up as well as referrals for laboratory, radiologic and special diagnostic studies. It includes consultative discussion regarding specialty referral. It includes ongoing medical management of medical conditions diagnosed. For conditions requiring specialist management, the appropriate referrals will be made. It includes prescription management within the scope of general internal medicine. It includes unlimited text, email and phone availability. It includes one office visit per month. It does not include emergency services or services within the rubric of “urgent care.” It does not include medical management of any condition which warrants hospitalization. It does not include infusion care, chemotherapy, radiation therapy, surgical services, radiologic services, laboratory services and other generally recognized medical diagnostic services.
